Email Request Form
To request a new email address for your hosted website, please fill out the form below.
In the Requested Email and Password box enter the name of the staff member, email userid, and password. Separate each with a space. If requesting more than one, enter the information for each userid on its own line.
Userids will be converted to all lowercase but the password will remain case sensitive. We recommend passwords be at least 8 characters long, contain a capitol letter, a number and a symbol (! @ # $ % & * . _ – + are all acceptable). Please do not include any spaces in the userid or password.

Jane Doe jdoe Nutr!t1onF@cts
Note: Library and Director name and contact email are required. The email id requested will automatically be assigned to your domain.
